About Life Care Hospice

Mission Statement

Our mission at Life Care Hospice is to provide a higher quality of care for all patients and their families when confronted with a life-limiting terminal illness. We recognize that death is a normal process in life and we strive for excellence in compassionate medical care. Our staff will serve as a voice for the terminally ill to help them live their final days with dignity while coping with grief and the end-of-life. Hospice affirms life and focuses on the quality of life. Hospice exists to provide support and care for persons in the last phases of an incurable disease so that they might live as fully and comfortably as possible.

Who is the Hospice Team?

The Hospice Team is comprised of an Interdisciplinary Care Team that focuses and designs an individual plan of care for each patient. Team members that will be involved in your care will be:

  • Attending Physicians
  • Hospice Medical Director
  • Registered Nurses
  • Licensed Practical Nurses
  • Home Health Aides
  • Licensed Social Workers
  • Chaplains
  • Volunteers

Life Care Hopice staff and volunteers unite with the community to accomplish a single goal:
