Life Care Hopice staff and volunteers unite with the community to accomplish a single goal:
To provide the patients in our Hospice Program and their families the highest quality of care possible so they may life each day to the fullest.
Standards of Excellence
- Admissions 24/7—our staff is available for a consultation or evaluation 24/7
- Post referral received : Contact patient/caregiver within one hour
- Nurse Practitioner on staff in each location
- Courtesy call every morning to follow up on all after hours calls/concerns
- Hospice Certified Pharmacist—Chris Borden
- Extensive Standing Orders—reduces the number of phone calls to the Attending Physician
- Upon Evaluation, if a patient is determined not eligible for the Hospice Benefit we will help facilitate Home health Referral
- Evaluate needs in home unrelated to patient diagnosis and facilitate assistance with needs
- Medication regimen not changed unless ordered by the attending Physician, attending Physician continues to facilitate patient care (attending Physician may transfer palliative care to Medical Director)
- Patient maintains pharmacy of choice
